FORMS INVENTORY

Forms are available for purchase from the PCBA.  The cost per printed page is $.20/page for members of the PCBA and $.50/page for non-members.  To determine the price of the forms the number of 8 1/2" x 11" pages for each form are listed next to the form name.  

You may pick up forms at our office or mail a note listing the forms you wish to purchase along with a check or money order for the forms and a self-addressed, stamped envelope for the PCBA to mail the forms back.  All checks and money orders should be made payable to "Pima County Bar Association".  If purchasing a packet or large amount of forms by mail, at least a 9" x 12" envelope is suggested.  DUE TO RECENT POSTAGE CHANGES, PLEASE CALL THE PCBA BEFORE SENDING A REQUEST FOR FORMS, PAYMENT AND SELF-ADDRESSED, STAMPED ENVELOPE TO DETERMINE THE POSTAGE NEEDED.
